What are the medical "have to's" for students entering LCCS? [Note: Most of these are required by the State of New Jersey]
Written evidence of:
- Up-to-date immunizations
- A physical exam by an MD, OD or Nurse Practioner
- Health history form for your child
- Check-off form regarding everyday minor medications
- If your child requires special consideration such as occasional
asthma medication during school or an Epi-Pen, it's very important to
contact the School Nurse to arrange for that care.
